Report Rules
- Do not store private client information, SSNs, account numbers, full tax returns, medical records, passwords, or payment cards.
- Use composite names unless the participant gave written permission.
- Capture exact words when useful, but remove anything personally identifying.
- Mark severity by customer harm, not by engineering difficulty.
- Every report should end with one recommended product change and one follow-up test.
