Report Rules

  • Do not store private client information, SSNs, account numbers, full tax returns, medical records, passwords, or payment cards.
  • Use composite names unless the participant gave written permission.
  • Capture exact words when useful, but remove anything personally identifying.
  • Mark severity by customer harm, not by engineering difficulty.
  • Every report should end with one recommended product change and one follow-up test.
